Leaving the Company Guide
As you take your next step in life, whether it be to retire and relax or pursue another career, we want to help make the transition of leaving the company a smooth one. This guide is to help you know what to expect as you manage the many changes and multiple tasks that are part of this process.

- You cannot use PTO to extend your employment. Associates that provide and work a 2 week notice will be paid for accumulated unused PTO that has been earned through the last day of work.
- Medical, Dental, Vision, Life and Disability coverage as well as Flexible Spending Accounts will end on your last day of employment with Cook & Boardman.
- Voluntary Life Insurance may be available. Contact HR for a portability form.
- Your Health Savings Account is available to you when you leave the company.
- You are always fully vested in the associate and employer contributions in your 401(k).
- Associates are responsible for all company property, materials, or written information issued to them or in their possession or control. All company property must be returned by associates on or before their last day of work.
- Prior to leaving the company, review and make any changes needed to your home address, personal email address, tax withholding elections, as well as your direct deposit account information. You can do this by going into your Employee Record information in Dayforce.
- Any post-employment payments paid by the company will be deposited to the account information on file at the time of payment. You will continue to have access to Dayforce through May of the following year so that you have access to your last pay stub and W-2 information.
- After you leave the company, you may make changes to your personal information by contacting HR@CookandBoardman.com. Please note that after you leave the company, any future changes to your contact information will need to be made directly with the HR vendor (401k, Medical, Dental, Vision, Life, etc.)
